Introduction 

This essay critically examines the care plan created for George, a 68‑year‑old gentleman recovering from a left‑sided stroke. It includes a brief patient overview, outlines the assessment method used, explains how priorities were set, and introduces the five key care issues addressed.

Patient overview

George recently suffered a moderate ischaemic stroke affecting his left side. He lives with his wife and struggles with mobility, dressing, and personal care. He is alert, understands his needs, and is motivated to recover.

Assessment approach

A holistic bio‑psychosocial assessment approach was used, combining the A‑E (Airway to Exposure) clinical checks with a broader evaluation of George’s physical, psychological, and social needs. This method was chosen to ensure we addressed not only George’s physical deficits but also his emotional state, social support, and functional needs at home.

Identifying problems

From the assessment, five priority concerns were identified:

1. Left‑sided weakness affecting mobility

2. Risk of falls

3. Difficulty in personal hygiene

4. Communication difficulties due to slight slurred speech

5. Risk of social isolation and low mood

These were ranked using the Maslow‑based prioritisation tool, which gives priority firstly to basic physical needs (mobility/fall risk/hygiene), then communication, and finally social and emotional wellbeing.
