Summative Assessment

 

Assessment type: a 2000-word care study.

“You should critically reflect on your role as a nursing associate by using your clinical experience and the principles of person-centred care to demonstrate how you have supported the planning, delivery and evaluation of high-quality patient centred holistic care for a patient you looked after.  This should include partnership with service users, families and the wider healthcare team to support the diversity in individual needs.”

You should select a health condition other than the one you did in your previous module NS4013. So, if you did your previous essay on Diabetes for NS4013, then you cannot use the same condition.

When you submit your summative assessment for NS5013, please include your topic used for your NS4013 as an appendix.

Weighting: 100%. Pass mark: 40%.

Module learning outcomes

 

You will need to demonstrate you meet the following module learning outcomes:

1. Critically analyse the principles of holistic person-centred care and evidence-based practice across health and social care.

2. Critically reflect on the role of the nursing associate as working in partnership with service users, families and the wider healthcare team to support the diversity in individual needs.

3. Explore strategies for assessing, planning and implementing care pathways across a range of clinical scenarios and debate the patient voice.

5. Demonstrate safe, effective nursing practice in a relevant clinical scenario, actively supporting self-care.

Work must be submitted through the VLE Moodle site.

Formative Draft Submission Deadline:Tuesday, 29th September 2020 between 0930 and 1600 to ensure the availability of support.

Summative Submission Deadline:Tuesday, 3rd November 2020 between 0930 and 1600 to ensure the availability of support.                 

DDS and extenuating circumstances deadline: 10th November 2020 between 0930 and 1600 to ensure the availability of support.

Summative Feedback will be given to students via VLE Moodle by 24th November 2020.

In Year retrieval date: Resubmission on Tuesday 8th December 2020 between 0930 and 1600 to ensure the availability of support.

Introduction:

150 words

1-     What you are going to write about? including description of the patient care setting

2-     Identify a care study – contextualize the content/ patient details e.g patient from ethnic background.

3-     State which reflective framework and/or theories you will be using on this care study.

4-     Which principles of Patient centred care you will use and how you will write a critical reflection of your role as a nursing associate in the addressing the need of that patient. 

5-     You want to demonstrate your knowledge, skills and attitudes in the whole process of Assess, Plan, Implement, Evaluate.

1-     You need to identify the needs of the patient in your care study example but focus on ONE main nursing problem.

2-     Give some further background of the patient which would also impact on the main problem.

3-     By focussing on ONE problem, you should be able to give reasons and support your actions with evidence. (e.g pain management; reduced mobility). How did you assess and what evidence-based approach you used?

Planning

500 words

1-     A critical exploration of the patients’ attitudes and beliefs of their health condition using the health belief model.

2-     Identification of the sociological dimension of health that includes health inequalities and health behaviours.

Implementation

500 words

3-     On reflection how did a specific theoretical framework and evidence- based approach influence your practice to deliver high-quality patient centred holistic care for a patient.

4-     This should include partnership with service users, families and the wider healthcare team to support the diversity in individual needs.

Evaluation

400 words

1-     Evaluate just that nursing problem but relate it to the overall outcomes of patient care.

2-     How did the condition you are writing, and your input impacted on the patient and their family/carers?

3-     How would you critically evaluate the influence you had on their experiences?

Conclusion

100 words

1-     Conclude with clear evidence-based recommendations concerning the use of this person-centred approach.

2-     What have you learned from this as a Nursing Associate? 

3-     How has your learning changed your knowledge, skills and behaviours?
