What impact will your plan have on the patient and their family? (consider what affect having so many health professionals being involved would have on your patient – confusion? / time of visits / impact on daily routine

 

What is required to reduce risks to the patient? – Team working/ communication/ leadership/ documentation/ MDT meetings/ proactive planning.

 

Consider relationships / empowerment / reducing risks / reducing re-admission / quality of life / follow up from several teams/ wellbeing (mental and physical)
