The Portfolio will be based on a series of tasks, linked to your chosen case study.

The focus will be on the biopsychosocial needs of the service user and how to enhance positive ageing.

You will be presented with 5 different case studies; you need to select one to be the focus of the portfolio.

You need to approach the portfolio from the perspective of the biopsychosocial model of holistic health and wellbeing assessment and planning, with an emphasis on person-centred care.

Remember this is Level 6 and you are required to critically engage with and analysis the theories and literature relevant to your chosen case study.
