Improving the Recognition and Escalation of Acute Patient Deterioration in Adult Medical Wards

Introduction and Topic Identification

This conference style presentation outlines an action plan for my Critical Review of Practice module. The chosen topic focuses on improving the recognition and escalation of acute patient deterioration within adult medical wards. This presentation critically explores my motivation for selecting this topic, outlines my initial thoughts prior to literature review, and considers the anticipated personal, professional, and organisational impact of this area of practice.

Early recognition of deterioration is a fundamental aspect of safe adult nursing practice and is directly linked to patient outcomes, staff confidence, and organisational safety culture. Despite the presence of structured tools such as the National Early Warning Score 2, failures in escalation and delayed responses continue to be identified in national reports. This makes the topic both clinically relevant and professionally significant at Level 6 practice.

All patient, staff, and placement details have been anonymised in line with professional codes of practice.

Problem and Issue Statement

Significance to Healthcare

Failure to recognise and respond to acute deterioration remains a persistent issue within healthcare systems. National data from NHS England and the Royal College of Physicians continues to show that preventable harm and avoidable cardiac arrests are often preceded by missed or unacted-upon warning signs. These include abnormal observations, subtle changes in patient condition, and staff uncertainty regarding escalation pathways.

From a healthcare systems perspective, ineffective escalation contributes to increased length of stay, unplanned intensive care admissions, and higher mortality rates. It also places additional strain on already pressured acute services, impacting sustainability and workforce wellbeing.

Significance to Adult Nursing Practice

Within adult nursing, responsibility for recognising deterioration often sits with registered nurses at the bedside. This requires not only technical competence in observation and assessment but also professional confidence, clinical judgement, and effective communication. In practice, escalation can be influenced by hierarchical barriers, fear of criticism, workload pressures, and inconsistent supervision.

Adult nurses are expected to act as patient advocates. Delayed escalation compromises this professional duty and conflicts with the NMC Code, particularly in relation to prioritising people, preserving safety, and practising effectively.

Initial Thoughts Prior to Literature Search

Before engaging with the literature, my initial view was that failures in escalation were primarily linked to individual confidence and experience levels. I believed that junior staff often recognised deterioration but felt reluctant to escalate due to fear of appearing incompetent or overreacting.

I also suspected that although tools like NEWS2 are widely implemented, their effectiveness depends heavily on ward culture, supervision, and leadership rather than the tool itself. These assumptions informed the direction of my literature search but remained open to challenge through evidence.

Motivation for Topic Choice

Personal Motivation

My interest in this topic developed through placement experience in adult medical wards, where I observed occasions where patient deterioration was recognised late or escalation was delayed. In one anonymised scenario, escalation occurred only after the patient’s condition had significantly worsened, leading to an emergency response that may have been preventable.

These experiences prompted reflection on my own confidence in escalation and highlighted the emotional impact on nursing staff when adverse events occur. This personal engagement has driven a desire to better understand how practice can be improved through evidence-based strategies.

Professional Motivation

Professionally, this topic aligns closely with my future career aspirations within acute adult nursing. Developing confidence in recognising and escalating deterioration is essential for safe autonomous practice as a registered nurse. It also links directly to leadership, communication, and supervision, which are key expectations at Level 6.

Exploring this topic supports my transition from student to registered practitioner and contributes to my readiness for employment.

Intention of the Study

My intention is an open enquiry rather than seeking evidence to support a predetermined solution. While I anticipate that education, supervision, and culture will be influential factors, I remain open to findings that may challenge my assumptions.

The aim is to identify evidence-based strategies that improve escalation practices while considering feasibility within real clinical settings.
