Effectiveness and Limitations of Interventions
While opioid substitution therapy has strong evidence supporting its effectiveness, it is not without criticism. Some argue that it replaces one dependency with another and does not address deeper social causes. However, research consistently shows that it reduces mortality, crime, and health risks.
Psychosocial interventions are effective but require sustained engagement, which can be difficult for individuals facing housing instability or mental health issues. Criminal justice approaches, such as imprisonment, have been shown to be largely ineffective in reducing long-term drug use and may increase harm by disrupting treatment continuity.

Countries such as Portugal offer an alternative model. Portugal decriminalised drug possession in 2001 and shifted focus towards treatment and social reintegration. Evidence indicates reductions in overdose deaths, HIV transmission, and drug-related stigma (EMCDDA, 2021).
Switzerland’s heroin-assisted treatment programme provides pharmaceutical-grade heroin under medical supervision for individuals who have not benefited from conventional treatments. This approach has led to improved health outcomes and reduced criminal activity.
These international examples suggest that health-led, compassionate interventions are more effective than punitive models.
